  1. In case of death
      • Accident report (Certified copy),
      • Family's civil registry statement,
      • Death certificate and autopsy report, if available,
      • Burial license,
      • Certificate of inheritance.
  2. In case of disability
      • Accident report (Certified copy),
      • Disability report specifying the disability percentage (from a general hospital),
      • A letter indicating whether the person has received payment from the Social Security Institution or not.
  3. In case of injury
      • Accident report (Certified copy),
      • Original copies of documents attesting treatment costs (invoices, prescriptions, receipts, drug barcodes),
      • Epicrisis issued by the hospital,
      • Bank account details.
      • If deemed necessary, the Insurance Company may request supplementary/additional documents.
