- Medical report specifying the disability percentage (from a general hospital),
- A letter indicating whether the person has received payment from the Social Security Institution or not.

In case of injury, the following documents shall also be required
- Original copies of documents attesting treatment costs (invoices, prescriptions, receipts, drug barcodes),
- Epicrisis issued by the hospital,
- Bank account details.
If deemed necessary, the Insurance Company may request supplementary/additional documents.

- Traffic accident report (Certified copy),
- Document certifying the status of the injured persons in the vehicle. Passenger? Bus attendant? Driver? (If not specified on the reports),
- List of passengers and the ticket belonging to the injured passengers,
- Official document attesting the employment of the driver and the bus attendant.
